Getting the Medical Marijuana Card for Florida

You will need to bring with you proof of residency such as a state ID, driver's license or passport.

If you are a seasonal resident of Florida:

• you must provide proof that you reside in Florida for at least 31 consecutive days in a calendar year.
• If you have a Florida ID card or driver's license, you could get approved immediately.
• If you do NOT have a Florida ID card or driver's license, then you have to wait for the state to approve you (which could take 7-14 days).

The Medical Marijuana Card is renewed annually and costs $77.75.

Getting the Medical Marijuana Card for Pennsylvania

Please be advised that the Pennsylvania Medical Marijuana Registry requires you to set up a profile before seeing the physician for a recommendation.

• Please go to: https://padohmmp.custhelp.com/
• From the Patients area, select Adult Patient Registration.
• Be sure that your name matches what is shown on your state issued ID.
• Once you see the physician and if you are approved for a recommendation, the physician will complete their piece of the registration.

The Medical Marijuana Card is renewed annually and costs costs $50+Tax.
